Osteoporose bei Nierenerkrankung

Ein Rückblick auf das Jahr 2019 mit Fokus auf Anti-Sklerostin-Therapie

Zusammenfassung

Mit abnehmender Nierenfunktion steigt das Frakturrisiko einschließlich hüftnaher Frakturen stark an. Zugrunde liegt die renale Osteopathie (auch renale Osteodystrophie genannt), eine sekundäre Osteoporose. Renale Osteopathie kommt in verschiedenen Phänotypen vor: vom stark erhöhten Knochenstoffwechsel („high turnover“) bis hin zum niedrigen/fehlenden Knochenstoffwechsel („low turnover/adynamic bone“). Eine One-size-fits-all-Therapie der renalen Osteopathie ist daher nicht möglich. Sklerostin ist ein Schlüsselmolekül im Knochenstoffwechsel. Der unlängst zugelassene Anti-Sklerostin-Antikörper Romosozumab inhibiert die Wirkung von Sklerostin und hat somit eine osteoanabole Wirkung. Ob eine Anti-Sklerostin-Therapie wie Romosozumab einen Stellenwert in der Behandlung der renalen Osteopathie einnehmen kann, wird im vorliegenden Artikel diskutiert.
